The Signature Well Man Blood Test

Both share the same core of thyroid, liver, kidney, lipid, HbA1c, iron, inflammation, hormone, vitamin and full blood count markers. The Signature adds nine that Premium does not have: PSA for prostate health, cortisol, and the advanced cardiovascular set of lipoprotein(a), ApoB, ApoA1, the ApoB to ApoA1 ratio, PLAC activity, plus transferrin saturation and the triglyceride to HDL ratio. Premium includes copper, ceruloplasmin and non-HDL cholesterol, which the Signature Well Man does not. If prostate health and a deeper look at heart risk matter to you, choose the Signature; if not, Premium covers the rest for £126 less.

Standard cholesterol tests measure how much cholesterol is carried in your blood. ApoB counts the number of particles carrying it, which is a better predictor of heart attack and stroke risk, because many small particles do more damage than a few large ones. Lipoprotein(a) is an inherited form of LDL that roughly one in five people carry at high levels; it raises heart risk independently of diet and lifestyle, does not change much over time, and is usually only worth measuring once. PLAC activity measures an enzyme linked to inflammation inside artery walls. Together they tell you whether your heart risk is higher or lower than your ordinary cholesterol suggests. Your GP report explains what to do if they are raised.

PSA is a protein made by the prostate. A raised level can indicate an enlarged prostate, inflammation or prostate cancer, and the NHS uses it as the first step before an MRI. A normal PSA is reassuring but not a guarantee, and a raised one is usually repeated and interpreted alongside your age and symptoms before any referral. Avoid vigorous exercise, cycling and ejaculation for 48 hours before the test, as they can push PSA up. Cortisol is the body's main stress hormone. A very high or very low level can point to an adrenal or pituitary problem, and a morning level gives the clearest reading, which is one reason we recommend a morning appointment for this test. Alongside testosterone, thyroid and HbA1c, cortisol helps explain persistent tiredness, poor sleep, weight gain around the middle and difficulty recovering from training.

Most thyroid tests measure TSH alone. This test measures TSH with free and total T4 and T3, so it shows both the signal from the pituitary and how much active hormone is in circulation, and it adds thyroid peroxidase and thyroglobulin antibodies, which reveal autoimmune thyroid disease years before TSH moves. That matters if you have a family history of thyroid problems, unexplained fatigue, or a borderline TSH result you want explained.

Choose the Signature if you want the advanced heart-risk markers and lipoprotein(a), a full thyroid panel with antibodies rather than TSH and free T4 alone, cortisol, DHEA-sulphate and the complete hormone profile. It suits men over 40, men with a family history of heart disease or early heart attacks, and men who want the most detailed baseline available for tracking over time. The Advanced Well Man at £175 covers PSA, testosterone, HbA1c, lipids, liver, kidney, iron, vitamins and a full blood count and is the better value if you do not need those extras.

Yes. We ask you to fast for 8 to 12 hours before your sample is taken, so book a morning appointment and have nothing but water from the evening before. Fasting keeps the triglyceride and glucose-related results comparable with the reference ranges. Take any regular medication as normal unless your GP has told you otherwise, and eat as soon as the sample is done.
