What is the Stockholm3 test?
Stockholm3 is a blood test for the early detection of aggressive prostate cancer. Instead of relying on PSA alone, it measures five proteins in the blood, including PSA and free PSA, analyses more than 100 genetic variants linked to prostate cancer risk, and combines those with your age, family history and any previous biopsy result. The output is a single risk score with a clear recommendation: refer for further investigation, or re-test in two to six years. It was developed in Sweden, has been used in routine care in Sweden, Norway and Finland since 2019, is included in European prostate cancer guidelines and has been evaluated in more than 75,000 men.
How is Stockholm3 different from a PSA test?
PSA measures one protein, and it misses cancers and raises false alarms in roughly equal measure: around 30 to 50 percent of aggressive prostate cancers occur in men with a PSA below 3 ng/ml, while three in four men with a raised PSA turn out not to have cancer. Stockholm3 adds four more protein markers, genetic risk and your clinical history, so it finds high-risk men that PSA alone misses, identifies about twice as many aggressive cancers in routine use, and reduces the number of men sent for MRI or biopsy who do not need it. It also tells low-risk men how long they can safely wait before re-testing.
Who should have the Stockholm3 test?
Men aged 45 to 74 who want to be screened for prostate cancer, particularly men with a family history of prostate cancer, Black men, whose risk is about double, men who have had a borderline or rising PSA and want a clearer answer before deciding on an MRI or biopsy, and men with a normal PSA who want the reassurance of a more complete risk assessment. It is not designed for men who have already been diagnosed with or treated for prostate cancer, who should be monitored by their specialist.
What does the Stockholm3 result look like?
You receive a detailed report with your risk score for clinically significant prostate cancer and one of two recommendations. Elevated risk: referral to a urologist for further examination, usually a prostate MRI, which our Prostate MRI page describes and which you can book without a referral. Low or normal risk: no further action now, and re-testing in two to six years depending on your score. The report also shows your PSA value. A Goodbody GP can talk you through it if you would like.
Does Stockholm3 replace a prostate MRI or biopsy?
No, it decides who needs one. An MRI shows what the prostate looks like; Stockholm3 tells you how likely it is that a scan will find something that needs treating. Using Stockholm3 before MRI means fewer men have scans and biopsies for cancers that would never have harmed them, while the men at genuine risk are found earlier, including some whose PSA is normal. If your score is elevated, MRI is the usual next step.
How accurate is Stockholm3?
In the Stockholm3 validation studies, replacing PSA with Stockholm3 found the same number or more clinically significant cancers while reducing biopsies by around a third and reducing the detection of harmless, low-grade cancers. In a 2024 UK-relevant analysis across ethnic groups it performed consistently in Black, Asian and white men. No screening test finds every cancer, and a low score does not guarantee you will never develop prostate cancer, which is why the report suggests a re-test interval.
Do I need a PSA test first?
No. Unlike some newer prostate tests, Stockholm3 measures PSA itself as one of its five protein markers, so you do not need a recent PSA result to take it. How do I prepare, and how is the sample taken?
The test needs a venous blood sample taken at a Goodbody clinic; it is not available as a home kit. No fasting is needed. Because PSA is part of the test, avoid vigorous exercise, cycling and ejaculation for 48 hours beforehand, and tell us if you have had a urine infection or a prostate examination in the previous few weeks, as these can raise PSA. You will complete a short questionnaire on age, family history and any previous biopsy, which feeds into the score.
How much does the Stockholm3 test cost in the UK?
£595 at one of 250+ Goodbody clinics, including the blood draw, laboratory analysis, the detailed risk report and the option of a GP follow-up. It is not currently available on the NHS, so private testing is the route for most men. For comparison, a PSA test alone is £69 and the EpiSwitch PSE test, which needs a PSA result within the last six months, is from £729.
How long do the results take?
Up to 14 working days from your sample reaching the laboratory. Stockholm3 is analysed by a specialist laboratory and the genetic markers take longer than a standard blood test. Your report is emailed to you and can be shared with your GP or urologist.
