What is Guardant360?
Guardant360 CDx is a comprehensive genomic profiling test performed on a blood sample rather than a piece of tumour. Cancers shed fragments of their DNA into the bloodstream; the test captures this circulating tumour DNA and sequences 74 genes to find the specific mutations driving the cancer. Some of those mutations have approved targeted drugs or immunotherapies that work against them, and others point to clinical trials. It is approved by the US FDA as a companion diagnostic and is used in the NHS for lung cancer. The result is a report your oncologist can use to choose or change treatment.
Who is Guardant360 CDx for?
People with a confirmed diagnosis of advanced solid cancer, stage III or IV, in situations where knowing the tumour's genetic profile could change treatment: the cancer has progressed on the current therapy, no tissue sample is available or a biopsy is not safe, or tissue testing came back incomplete or inconclusive. You need to have been off treatment for at least 10 days so that enough tumour DNA is circulating. It is not suitable for blood cancers or for early-stage disease, and it is not a screening test for people without a diagnosis.
Is Guardant360 available in the UK, and can I get it on the NHS?
Yes, it is available privately through Goodbody Clinic across the UK. The NHS uses Guardant360 for some patients with advanced non-small cell lung cancer, where tissue is unavailable, through the Genomic Medicine Service; for other cancers access is limited. If your oncologist has recommended comprehensive genomic profiling and it is not available to you, or waiting for it would delay treatment decisions, private testing is a way to get the same report within about two weeks of booking.
How much does a Guardant360 test cost in the UK?
From £2,469 with your own blood collection, £2,499 at a Goodbody clinic or £2,529 with a nurse home visit. The required consultant oncologist consultation is £149 and is refunded against the test if you proceed, so the consultation costs nothing if the test goes ahead. The price includes sequencing of all 74 genes, the clinical report and a results consultation with the oncologist. We also offer Guardant360 Liquid, a 740-gene panel with epigenomic analysis, from £4,320.
Why is an oncologist consultation required first?
Because the test is only useful in specific circumstances, and the report is only useful if someone can act on it. Our consultant oncologist reviews your diagnosis, stage, previous tissue results and treatment history to confirm the test could change your options, checks you have been off treatment long enough for a reliable result, and advises whether Guardant360 CDx or the larger Guardant360 Liquid panel is the right choice. The consultation is a 30-minute telephone call, does not replace your own oncologist, and is refunded if you go ahead with the test.
How does a liquid biopsy compare with a tissue biopsy?
A tissue biopsy examines a sample from one site of the tumour, needs a procedure that may involve anaesthetic, radiation guidance or surgery, and can take weeks. A liquid biopsy needs two tubes of blood, carries no procedural risk, reflects DNA shed from all tumour sites, and reports within a week. Its limitation is that some tumours shed little DNA, in which case the result may not detect mutations that are present; in those cases a tissue test may still be needed, and the report says so. The two are complementary, and oncologists increasingly use blood first.
What does the Guardant360 report show?
A list of the genomic alterations detected in your tumour DNA, mutations, fusions, amplifications and, where relevant, markers such as microsatellite instability, matched to approved targeted therapies and immunotherapies for your cancer type and to relevant clinical trials. It also reports the fraction of tumour DNA found, which indicates how much confidence to place in a negative result. Our oncologist walks you through the report and prepares a summary for your treating team.
How long does the process take?
Book the oncologist consultation, usually available within days. If the test is suitable, book your blood draw at one of 250+ clinics or with a nurse at home. Results are returned within 7 working days of the sample reaching the laboratory, and your results consultation is booked as soon as they arrive. Most people have a report in hand within two to three weeks of first contact.
Will my own oncologist accept the results?
Guardant360 CDx is the same test used in NHS and international practice and its reports are written for oncologists, so your treating team can act on them directly. Our oncologist provides a summary letter to accompany the report. Decisions about changing treatment remain with the team treating you.
Does Guardant360 detect all cancers, or tell me if I have cancer?
No. It is not a screening test and should not be used by anyone without a confirmed cancer diagnosis; for screening, see our TruCheck test. It profiles the DNA of a known solid tumour to guide treatment. It does not cover blood cancers.
What if no actionable mutation is found?
Around one in three profiles finds no alteration with an approved matched therapy. That is still useful: it rules options out, may identify trials, and tells your team that standard treatment remains the best path. If the tumour DNA fraction was very low, the oncologist will explain whether a repeat or a tissue test is worthwhile.

What happens next
Your results are emailed within the stated turnaround of your sample reaching the laboratory, and are reviewed with you by our consultant oncologist, who explains what the findings mean and sets out the options to discuss with your treating team. Your report is written for oncologists and can be shared with your own specialist. All samples are analysed by an accredited laboratory.
