What is Guardant360 Liquid, and how is it different from Guardant360 CDx?
Both are liquid biopsies that profile tumour DNA from a blood sample to guide treatment of advanced solid cancer. Guardant360 CDx sequences 74 genes, the ones with approved targeted therapies, and is the established, FDA-approved panel. Guardant360 Liquid, built on the GuardantINFINITY platform, sequences more than 740 genes and adds more than 19,000 epigenomic signals, so it can find rarer alterations, resistance mechanisms and trial-eligible targets that a 74-gene panel does not cover, and it detects variants at lower levels. Our oncologist advises which is the right test for your situation; for many people CDx is sufficient, and Liquid is chosen when the search needs to be as broad as possible.
Who is the extended panel for?
People with a confirmed advanced solid cancer where a broader search could change treatment: when CDx or tissue testing found nothing actionable, when the cancer has progressed on a targeted therapy and the resistance mechanism matters, when a clinical trial is being considered, or when a rare cancer type makes a wide panel more informative. As with CDx, you need to be at least 10 days off treatment. It is not for blood cancers, early-stage disease or screening.
What do the epigenomic signals add?
Genomic sequencing finds changes in the DNA code. Epigenomic analysis looks at methylation, chemical marks that switch genes on and off, which cancers alter extensively. Including 19,000 methylation signals improves the detection of tumour DNA when little is present, helps distinguish tumour DNA from age-related changes in normal blood cells, and can reveal how a cancer has adapted to treatment. It is the reason this panel is described as multi-dimensional rather than simply larger.
What does 0.001 percent sensitivity mean in practice?
Circulating tumour DNA is a tiny fraction of the DNA in blood; the rest comes from normal cells. Detecting a variant at 0.001 percent mutant allele fraction means the test can find one tumour-derived fragment among 100,000 normal ones. That matters when the tumour sheds little DNA, for example after partial response to treatment, where a less sensitive test might report nothing.
Why is an oncologist consultation required first?
To make sure the test can change something. Our consultant oncologist reviews your diagnosis, stage, previous genomic results and treatment history, confirms you have been off treatment long enough, and advises whether Guardant360 Liquid or the smaller CDx panel is the better choice. The consultation is a 30-minute telephone call, costs £149, is refunded if you proceed, and does not replace your own oncologist.
How much does the Guardant360 Liquid test cost?
From £4,320 with your own blood collection, £4,349 at a Goodbody clinic or £4,380 with a nurse home visit, including sequencing, epigenomic analysis, the clinical report and a results consultation with our oncologist. The £149 consultation is refunded against the test. The 74-gene Guardant360 CDx is from £2,469.
What does the report show?
The genomic alterations found, mutations, fusions, amplifications and biomarkers such as tumour mutational burden and microsatellite instability, matched to approved therapies and clinical trials for your cancer type, plus epigenomic findings and the tumour DNA fraction, which indicates how much weight a negative finding carries. Guardant updates the panel's features over time at no extra cost. Our oncologist walks you through the report and writes a summary for your treating team.
How long does the process take?
The oncologist consultation is usually available within days. Results are returned within 7 working days of the sample reaching the laboratory, and your results consultation is booked as soon as the report arrives. Most people have a report in hand within two to three weeks of first contact.
Is this available on the NHS?
Not as a 740-gene panel. The NHS Genomic Medicine Service offers smaller targeted panels for specific cancers, and Guardant360 CDx for some lung cancer patients. Private testing is the route to the extended panel in the UK.
Can this test tell me if I have cancer?
No. It is a treatment-guidance test for people with a confirmed diagnosis of advanced solid cancer, not a screening test. For screening without a diagnosis, see our TruCheck test.

What happens next
Your results are emailed within the stated turnaround of your sample reaching the laboratory, and are reviewed with you by our consultant oncologist, who explains what the findings mean and sets out the options to discuss with your treating team. Your report is written for oncologists and can be shared with your own specialist. All samples are analysed by an accredited laboratory.
